Zeroing in on little mouths: El Rio increasing child dental care to head off host of ills

Feb. 3--Failure to thrive, high fevers and cellulitis are some of the more serious yet common results kids suffer when they have undiagnosed dental problems.

"I've had a kid with a dental abscess who had a fever of 105 rushed to the hospital," said Dr. Ray Wagner, a pediatrician at Tucson's El Rio Community Health Center, which is stepping up its dental services to at-risk children this year.

"There are kids with failure to thrive; they are not gaining weight because they can't chew their food properly. Good oral health is an important part of a child's health and well-being."

A pilot dental program that El Rio began last year -- offering free protective sealants and fluoride varnishes to children -- is expanding in 2009 by adding more hygienists on more days. Ideally, officials with the center say they would like to have a dental hygienist giving preventive treatments and offering risk assessments at kiosks in all of its pediatric locations.

The new effort is in part to help implement a recent recommendation by the American Academy of Pediatric Dentistry that children be seen for their first dental visit within six months of their first tooth appearing.

El Rio, which has 16 sites in Pima County, last year served 75,000 county patients. Half of those patients were enrolled in the Arizona Health Care Cost Containment System, Arizona's version of Medicaid.

Early childhood tooth decay, often known as "baby bottle mouth," is the most common chronic disease of children. About 59 percent of school-age children have the disease, whereas 19 percent of school-age children are obese and 11 percent have asthma, Wagner said.

It's not unusual for El Rio dental hygienist Cristina Montiel to see children who appear to never brush their teeth.

"I see gingivitis, abscesses," said Montiel.

Using a purple "disclosing solution," Montiel is able to show kids and parents their "sugar bugs" -- areas where bacteria has built up and is threatening the tooth.

Maydorena Salcido, 36, was at El Rio for a medical appointment Monday but was happy to bring her 13-month-old daughter, Missel Salcido, to get the screening and varnish on her tiny teeth.

Salcido said she waited too long with her eldest son, now 7. She didn't bring him to the dentist for his first visit until he was 4, and he's since had to go through root canals and has crowns on some of his teeth.

"If we don't see a kid until they are 3 or 4 years old, the damage is done already," said Dr. Greg La Chance, who is El Rio's dental director. "The enamel on a baby's tooth is thinner, and it doesn't take much to get through to the underlying tooth structure."

La Chance said El Rio wanted to expand its pilot program because the screening and prevention work has helped give its patients a "dental home" where they will return for checkups.

"When you've been in this profession long enough and seen all kinds of misery, you want to do what you can to do prevention," he said. "We hope as we go along with this program to kind of show how it's affecting our patient population. Hopefully, we will see a decrease in decay rates."

In a waiting room with an aquarium and a bright blue wall decorated with purple fish, younger kids have their teeth cleaned by a hygienist and rinse with the disclosing solution, and then receive a sticky, shiny "fluoride varnish" that's supposed to protect the teeth. They get a toothbrush as a reward. Older kids receive a sealant that fills in gaps and pits in the teeth, also to prevent cavities.

Wagner said in some severe cases the decay is so bad that children have a mouthful of teeth that become nothing but brown stumps that must be pulled.

"In the past it wasn't recommended to see a dentist until the age of 3. A lot of kids by the time they got to the dentist had a lot of tooth decay happening," he said. "Tooth decay in children is a really big expense to society, and most pediatricians have never been educated to look for it."

The American Academy of Pediatrics, the American Academy of Pediatric Dentistry and the American Dental Association have recommended children have a "dental home" by the age of 1.

"Part of our goal is to integrate dental and medical care," said Nancy Wexler, administrator of El Rio's dental department. "Everyone talks about integrated medical care, but the mouth gets left out."

Rift over mouth rinse and cancer

Results of a review published in the Australian Dental Journal found there was "sufficient evidence" that "alcohol-containing mouthwashes contributed to the increased risk of development of oral cancer".

Lead author of the review Michael McCullough called for mouthwashes containing alcohol to be removed from supermarket shelves and reclassified as "prescription only".

But the head of the University of Queensland's School of Dentistry, Laurence Walsh, said yesterday that mouthwash was more likely to help prevent cancer than bring it on.

"There is a whole range of reasons why some mouth rinses would actually reduce the chance of the cancer because they impair the production of molecules that do have cancer-causing effects like acetaldehyde," Professor Walsh said.

"There is literature which is now recommending that mouth rinses be used to reduce bacteria which produce acetaldehyde and there is a fairly strong consensus internationally that mouth rinses containing alcohol aren't linked to cancer."

Dr McCullough argues that acetaldehyde, a toxic by-product of alcohol that is believed to be carcinogenic, may accumulate in the oral cavity when mouthwash is used.

He said yesterday that he had never heard of any studies showing the molecule could suppress or prevent cancer.

Oncologist Andrew Dean, of Perth's St John of God Hospital in Subiaco, said oncologists would warn patients of the potential risk of using alcohol mouth washes and encourage further research in the area, as a result of the review.

"We caution people against using a powerful alcoholic-based mouthwash on a daily basis and in particular a subset of the population who use mouthwash to mask the smell of cigarette smoke," Dr Dean said.

He urged people to report anything unusual in their mouths, such as unexplained lesions, to their dentist or general practitioner and called for more research in the area.

Keep your baby smiling with proper, early dental hygiene

Nearly every baby book on the market warns parents not to put children to bed with a bottle of milk, formula or juice. The practice can lead to tooth decay.

But Chansley, of Tacoma, Wash., had no idea that pediatricians and dentists also recommend that, after every feeding, she wipe daughter Cerridwen's tiny toothless gums with a clean, damp washcloth or piece of gauze.

Like many babies, Cerridwen likes to fall asleep after feeding. Says Chansley: "I can't see putting my hands in a sleeping baby's mouth"

But dental experts are urging parents to start dental hygiene at birth. While conventional wisdom once argued that baby teeth don't matter because they soon fall out, dentists now know that baby teeth set the stage for what is to come.

The Washington Dental Service Foundation, a nonprofit arm of dental insurer Washington Dental Service, has launched a campaign to draw attention to the care of baby teeth and gums. The foundation is recruiting both dentists and pediatricians as part of its effort. Since 2002, it has offered oral health education for physicians.

Dr. Ovidio Penalver, a Puyallup, Wash., pediatrician, is one doctor who's on board with the message. "Neglected baby teeth lead to problems with permanent teeth," he says. "Dental problems can lead to further problems that affect the whole health of a child."

Dental problems are especially acute among low-income children.

A 2005 survey published by the state Department of Health found that 45 percent of low-income preschoolers in Washington had dental decay, compared with only 28 percent of preschoolers nationally. In 2004, 45 percent of low-income preschoolers in Washington showed signs of tooth decay, compared to 38.3 percent a decade earlier.

Allowing children's dental issues to fester can result in pain, malnutrition, speech development problems and damage to self-esteem, Penalver warns.

In years past, parents were often advised to have their child's teeth checked by age 3. But new research has shown that earlier checkups can help prevent dental disease, according to the Washington Dental Service Foundation.

Experts, including the foundation and the American Academy of Pediatric Dentistry, now urge parents to have their babies' teeth examined by a dentist or pediatrician as soon as teeth appear, and at least by age 1. "When you have teeth, you have to take care of them," Penalver says.

Earlier oral checkups mean problems are spotted sooner. The exams are also a chance for a doctor or dentist to offer parents advice on caring for their baby's teeth and offer suggestions on diet and eating habits.

"All children go to their primary care medical office many times before age 3," says Dianne Riter, spokeswoman for the Washington Dental Service Foundation. "At that time, when the primary care provider is looking at overall health, it make sense for them to also look at teeth and assess the risk of decay."

Penalver believes primary-care doctors such as pediatricians and family physicians can play an important role. "Most young kids don't go to the dentist," he says, so it's up to doctors to step in and offer care. "You need to emphasize regular cleaning of teeth. And as soon as you can, flossing."

While the use of fluoride to prevent cavities can be controversial, with some arguing that ingesting fluoride is dangerous, doctors and dentists continue to recommend it.

Penalver suggests that parents learn whether their municipal water supply is fluoridated. Puyallup, for example, does not add fluoride to its water; Tacoma does.

"In areas without fluoride, children should have fluoride supplementation starting at 6 months (of age)," he says.

In addition, Penalver says, "we are starting to use fluoride varnish in our office as soon as they have teeth."

The varnish is a substance that is painted directly onto a child's teeth. A study published in 2006 in the Journal of Dental Research found that fluoride varnish treatment could cut children's cavity rates in half. Children in the study ranged from 6 months to 3 1/2 years old.

"It's something primary-care doctors can do in their office," Penalver says.


TIPS FOR PARENTS

--Beginning at birth, wipe your baby's gums with a washcloth or piece of gauze after feeding.

--As soon as you see baby's first tooth, at around 6 months of age, start brushing it with a soft toothbrush made especially for babies.

--Use a rice-sized amount of fluoridated toothpaste. Try putting your child's head in your lap to make brushing easier.

--Avoid constant snacking on sugary, sticky or starchy foods.

--Don't let your child sip sweet liquids throughout the day.

--Offer healthy snacks such as cheese, fruit or vegetables.

--If you put your baby to bed with a bottle, fill it with water. Formula, milk, juice or other liquids can increase your child's risk of cavities.

--Watch for white spots on the teeth or changes to gums. These can be early signs of problems.

The Town of Tonawanda couple were sailing along on his income as a plant worker and hers as a travel agent when he suffered a brain aneurism in late 2002. They suddenly had to get by on her paycheck, and by the time he returned to his job nine months later, were $27,500 in debt.

“When bills started piling up I tried making ends meet with credit cards,” Irene “Reenie” Piazza said. “It got to be too much.”

Staring at bankruptcy, with no obvious way out, the couple turned to Consumer Credit Counseling Service of Buffalo.

“We started them on a need-to-use budget. The first step: get rid of the credit cards,” said Kate Beck, the organization’s education coordinator. The Piazzas also enrolled in a financial literacy program that fit their circumstances.

It was a struggle, but by early 2006 they had regained their financial footing, and are cited as a success story by Consumer Credit Counseling. People seeking assistance can hear their story on a public service announcement recorded by the Piazzas that plays in the waiting room.

They’ll never be caught short again, even by an unpredictable event like sudden illness, Reenie Piazza said.

Rule No. 1 in her personal guidebook: “If you don’t need it, don’t buy it. Live within your means.”

Rules No. 2 and 3:

• “If you can’t make the full payment, always try to at least put something

toward the bill.”

• “Instead of going out, stick to home gatherings with family and friends. Do the cooking yourself.”

Coming from people like the Piazzas, who’ve been through the grinder and survived to tell about it, such suggestions might carry extra weight with anyone blind-sided by the current global economic crisis.

“Everybody is being impacted in some way, shape or form,” said Paul Atkinson, executive director of Consumer Credit Counseling. “They haven’t been punched in the nose like this in the past.”

Like the Piazzas, professional advisers have plenty of tips for staying financially upright. “You just need some tools,” Atkinson said. “Right after the first of the year is a great time to pick pick out a few and at least get started on the right foot.”

For example, Jesse Mecham, creator of “You Need A Budget,” recommends four rules for setting up a “living” budget:

• Stop living paycheck to paycheck. “Instead of living on what you currently earn, start living on what you earned the previous month,” Mecham advises.

• Give every dollar a job. “Every dollar you earn must be given a specific task. Money not given a job is wasted money.

• Prepare for rain. “Unexpected expenses can creep up anytime,” he notes. They should be taken into consideration “far enough in advance so you can stick to the budget.”

• Roll with the punches. “Developing and sticking with a budget can be difficult, but it is important. Make the needed adjustments from time to time to ensure success.”

In addition to budgeting, Scott Mitic of TrustedID suggests keeping sufficient cash reserves on hand — at least six months of income; saving by cutting unnecessary expenses; keeping track of your credit score by getting free annual credit reports from each of the big reporting agencies, Equifax, Experian and TransUnion; trying to prevent identity theft by keeping your Social Security number to yourself, shredding mail that contains account information or new credit card offers and changing your computer password at least monthly; and increasing building your investments through a diversified portfolio.

• Shopping for better deals on automobile and homeowners insurance. “If you have enough savings, consider increasing the deductibles in order to reduce premiums,” Lauber adds.

• Considering taking part-time work to augment your income.

• Consolidating debt.

• Planning meals for the whole week, and shopping accordingly. Try to choose recipes that will yield both dinner and a brown-bag lunch the following day.

• Checking your pay stub to see if too much tax is being withheld. “If you’ve been getting a large refund, talk to your tax adviser about reducing the withholding. It’s put more income in your paycheck.
